Dutasteride for hair loss: dutasteride vs finasteride in the Philippines

Quick answer

Dutasteride is a prostate drug that blocks both forms of the enzyme that makes DHT, so it lowers DHT more than finasteride. In a head-to-head trial, dutasteride 0.5mg grew more hair than finasteride 1mg at 24 weeks[1]. In the Philippines it is registered for the prostate, so hair loss use is off-label and needs a doctor who is willing to prescribe it.

What dutasteride is

Dutasteride (brand Avodart; Dutarid is another brand seen in PH pharmacies) is labelled for an enlarged prostate (BPH)[2]. Like finasteride, it is a 5-alpha reductase inhibitor. Community shorthand is dut, and finasteride is fin.

The enzyme that makes DHT comes in two forms, type I and type II. Finasteride blocks mainly type II[3]. Dutasteride blocks both[2]. That is why it lowers DHT further, and why dermatologists consider it when finasteride is not enough.

Dutasteride vs finasteride at a glance

The key trial randomized men with male pattern loss to several doses of dutasteride, finasteride 1mg or placebo. At 24 weeks, dutasteride 0.5mg increased hair counts more than finasteride 1mg[1]. Longer head-to-head data are limited, and the trial does not tell you how either drug performs in your scalp.

Side effects and the long half-life

The side effect list resembles finasteride: lower libido, erectile dysfunction (ED, trouble getting or keeping an erection), less semen, and breast tenderness or enlargement[2]. In healthy young men, dutasteride lowered semen volume and sperm counts over six months, with recovery after stopping[6].

The practical difference is time. Dutasteride has a half-life of about 5 weeks, so it stays in the body for months after the last dose[2]. If side effects appear, they may take longer to fade than with finasteride. Women who are or may become pregnant should not handle leaking capsules[2].

Like finasteride, dutasteride roughly halves PSA, the prostate cancer screening test, after about six months[2]. Tell any doctor ordering a PSA test.

Full finasteride numbers, including fertility and mood, are in the finasteride side effects guide. Every treatment fear is in the side effects hub.

Getting dutasteride in the Philippines

Dutasteride is sold at Mercury Drug and hospital pharmacies for the prostate. Users report Mercury staff asking about a prescription[9], and some clinics refusing to prescribe it for hair at all[5]. The telehealth brands we compared do not list it for hair. Price: [TODO: price for dutasteride 0.5mg at Mercury Drug].

  • Start with a dermatologist. A consult costs about ₱400 to ₱1,000, often free with an HMO[10]. Bring photos of your hair over time.
  • Ask why. A good derm will want to know how long you used finasteride, at what dose, and with what result.
  • Avoid reseller capsules. Dutasteride from Facebook or email sellers has the same verification problems as oral minoxidil. See is it legit?

Dutasteride with minoxidil

Dutasteride, like finasteride, slows DHT-driven loss but does not directly grow hair. Men who use it often pair it with topical or oral minoxidil, which works by a different route. One Filipino user posted progress on oral minoxidil plus dutasteride at month four[7]. Single progress posts show what is possible for one person, not what is typical.

Starting two drugs at once makes it hard to tell which one causes a side effect. That matters more with dutasteride, because it takes months to leave the body[2]. Many derms prefer to change one thing at a time.

Why some clinics say no

Users on r/PhHairloss report clinics that will not prescribe dutasteride for hair at all[5]. There are reasonable grounds for caution. Hair loss is not on its PH label, so the prescriber takes on off-label responsibility. Its long half-life means side effects cannot be switched off quickly[2]. And finasteride, which is labelled for hair, has longer track records in hair loss trials[4].

A refusal is not a judgement on you. Ask the doctor to explain their reasoning, and if you disagree, a second opinion from another PDS board-certified dermatologist is reasonable. Do not fill the gap with capsules from a reseller. Unlabelled capsules cannot be checked for dose, origin or expiry, and leaking capsules are a handling risk for anyone pregnant at home[2].

Questions to ask your dermatologist

  • Has my finasteride trial been long enough to call it a failure?
  • Is my loss definitely male pattern, or could something else be going on?
  • What dose and schedule would you use for hair, and why?
  • Which brand should I buy, and where can I fill it?
  • What should I watch for, and when should I come back?
  • If I plan to have children, how should I time this?
  • Should I tell my doctor before any PSA test or blood donation?

When derms consider it

Dermatologists usually consider dutasteride after a fair trial of finasteride, usually many months, has not stopped the loss. It is rarely a first step. Other options at that point include adding oral minoxidil, switching to topical finasteride if side effects were the issue, or reviewing the diagnosis.

For reference, finasteride 1mg (Stercia) runs about ₱1,800 a month. Compare everything on the prices page. The pillar guide is finasteride in the Philippines, and the full ladder is in hair loss treatment. For online prescribers, see online hair loss doctors.
