Can dandruff cause hair loss? Balakubak, seb derm and hairfall explained

Updated 27 Oct 2026Prices checked October 2026Scalp skinWhole scalp
Quick answer

Dandruff does not cause permanent baldness, but a flaky, itchy, inflamed scalp can make hairfall worse through scratching, breakage and inflammation[1][2]. Treating it with a medicated shampoo like ketoconazole, left on for about five minutes, usually calms both[3]. If flakes come with patches of missing hair, see a dermatologist, because that points to something other than ordinary balakubak.

Does dandruff make your hair fall out?

Not directly, and not permanently. Dandruff is the mild end of seborrheic dermatitis, a common skin condition driven by oil and a yeast called Malassezia that lives on everyone's scalp[1]. It causes flakes, itch and sometimes redness. It does not destroy hair follicles the way scarring conditions do.

What it can do is make hairfall worse in the short term. Constant itch leads to scratching, and rough handling breaks hair[2]. An inflamed scalp is also a poor environment for growing hair. Many people notice more hair in the drain during a bad flare, then less once the scalp settles.

Dandruff and pattern hair loss often happen together, because both are common and both are linked to oily scalps. If your hairline is moving back or your part is widening, treating the flakes will help comfort but will not stop the thinning. That needs its own treatment, covered in our hair loss treatment guide.

The dandruff shampoo routine that works

Filipino dandruff threads land on the same answer: ketoconazole (Nizoral or the cheaper Dermazole) or Selsun Blue, used properly[7]. Ketoconazole 2% cleared seb derm and dandruff in most patients in a large trial, and once-weekly use kept it from coming back better than placebo[8]. Selenium sulfide also beat placebo for moderate to severe dandruff[9].

  1. Wet your scalp fully. If it is very oily, wash once with regular shampoo first.
  2. Massage the medicated shampoo into the scalp, not the lengths.
  3. Babad: leave it on for about five minutes[3].
  4. Banlaw: rinse well and condition the ends if they feel dry.
  5. Use it two to three times a week at first[10], then once a week to keep flares away[8]. Use a gentle shampoo on other days.

Full brand comparisons, prices and how to spot fake Nizoral on Shopee are in our ketoconazole shampoo guide. For other shampoo types, see the best shampoo for hair loss.

Flakes after starting minoxidil

If the flakes started after you began topical minoxidil, the product may be the cause. Users on r/PhHairloss describe dandruff and itch in the first weeks of Regroe[11]. Many minoxidil liquids contain propylene glycol, a common cause of irritation and contact dermatitis[6]. Dried minoxidil residue can also look like dandruff. A medicated wash a few times a week often helps. If the scalp turns red, swollen or very itchy, stop and see a doctor. Switching to a foam without propylene glycol is one option to discuss; see the minoxidil guide.

Habits that make a flaky scalp worse

  • Skipping washes because of hairfall. Some women wash only every two or three days because the shedding scares them[12]. Hair that comes out in the shower was already shed[13]. A dirty, oily scalp flares more.
  • Heavy oiling. Coconut oil and rosemary oil massages are popular, but oil feeds the yeast behind seb derm. If you have flakes, keep oils off the scalp. See rosemary oil for hair.
  • Damp, covered hair. Tying up wet hair or wearing a helmet over a sweaty scalp all day keeps it moist and warm.
  • Scratching with nails. Use fingertips when washing. Scratching breaks hair and irritates skin[2].
  • Changing shampoo every week. Give one medicated routine four weeks before judging it[10].

When to see a dermatologist

See a PDS board-certified dermatologist if flakes spread to your eyebrows, sideburns, ears or chest, if the scalp is painful, weeping or crusted, if there are round patches of missing or broken hair, or if four weeks of a correct medicated routine change nothing[1]. One poster's flakes reached her eyebrows and sideburns after trying Sunsilk, Clear and Head and Shoulders[15]. That needs a diagnosis.

A consult costs ₱400 to ₱1,000, or nothing with an HMO at many accredited hospitals, users report[16]. You can find a derm through the Philippine Dermatological Society or book an online consult through NowServing. For combined scalp and thinning concerns, compare online hair loss doctors.

Questions people ask

Can dandruff cause hair loss?

It can increase shedding and breakage during flares through itch, scratching and inflammation[1][2], but it does not cause permanent baldness.

Will my hair grow back after treating dandruff?

Hair lost from a flare usually grows back once the scalp calms. Thinning from pattern hair loss will not, and needs separate treatment. See hair loss treatment.

What is the best dandruff shampoo in the Philippines?

Ketoconazole (Nizoral, Dermazole) and selenium sulfide (Selsun Blue) have the best evidence[8][9]. How you use it matters: leave it on for about five minutes[3].

Is seborrheic dermatitis the same as dandruff?

Dandruff is the mild form of seborrheic dermatitis. Seb derm can also cause redness and affect the face, ears and chest[1].

Should I stop washing my hair if it is falling out?

No. Hairs in the shower were already shed[13]. Washing less lets oil and flakes build up, which can make the scalp worse.
