Finasteride side effects: what the trials show, in plain language

Quick answer

In the main finasteride 1mg trials, sexual side effects such as lower libido and erectile dysfunction were reported by about 1 to 2 in 100 men, only slightly more than on placebo, and most resolved after stopping[1]. Rarer effects on the label include breast tenderness or enlargement, depression, and lower semen volume. If side effects appear, see a doctor instead of adjusting the dose yourself.

"Takot ako sa side effects" is the most common line in Filipino finasteride threads. Users on r/PhHairloss ask some version of "will it affect my manhood?" more than any other question[2]. This page gives the numbers from the trials and the label, then explains what to do if something feels wrong.

Sexual side effects: the trial numbers

Sides is community shorthand for side effects. ED means erectile dysfunction: trouble getting or keeping an erection firm enough for sex[3]. In the 12-month placebo-controlled trials of finasteride 1mg, the label reports these rates[1]:

Sexual side effects in the finasteride 1mg trials
Side effectFinasteride 1mgPlacebo
Decreased libido (lower sex drive)1.8%[1]1.3%[1]
Erectile dysfunction (ED)1.3%[1]0.7%[1]
Ejaculation disorder (including less semen)1.2%[1]0.7%[1]
Sexual side effects in the finasteride 1mg trials

Put plainly: out of 100 men taking finasteride for a year, roughly one or two noticed lower sex drive. About one noticed ED. On sugar pills, close to one in 100 reported the same things. The label says these effects went away in men who stopped, and in many who kept taking it[1].

The label also lists reports collected after approval, outside trials: sexual problems that continued after stopping, testicular pain, and male infertility or poor semen quality[1]. Post-approval reports do not tell us how often something happens, only that it has been reported. They are real enough to take seriously.

The nocebo effect

Expecting a side effect can cause it. In an Italian study of men taking finasteride 5mg for the prostate, 43.6% of men told about possible sexual side effects reported them, against 15.3% of men who were not told[4]. Same drug, same dose. The only difference was the warning.

This does not mean side effects are imaginary. It means worry is a real factor, and reading horror stories before your first dose can change how you feel. Users on r/PhHairloss say nocebo framing and peer "no sides" replies helped them start[5].

Fertility: nakakabaog ba?

Finasteride can lower semen volume, and in some men sperm counts. In a study of healthy young men, finasteride and dutasteride reduced semen volume and sperm counts over six months, and values recovered after stopping[6]. The label lists male infertility and poor semen quality among post-approval reports, with improvement after stopping[1].

Most men on finasteride father children without problems, but if you and your partner are actively trying to conceive, raise it with your doctor before starting or while trying. Users on r/PhHairloss ask this question often and it usually goes unanswered[7].

Mood, depression and brain fog

Depression is listed on the finasteride label from post-approval reports, and suicidal thoughts have also been reported[1]. These are uncommon, but they matter more than any hair outcome. One Filipino user described brain fog and lower sex drive[9]. Another switched to topical finasteride after suicidal thoughts at month six[10].

Some men report sexual, mental or physical symptoms that continue after stopping. This is sometimes called post-finasteride syndrome (PFS). The term barely appears in Filipino threads, and how often it happens is not established. The label does list persistent sexual problems after stopping[1].

Gynecomastia and breast changes

Breast tenderness and breast enlargement (gynecomastia) are listed on the label[1]. Male breast cancer has also been reported rarely[1]. One poster in r/PhHairloss described gynecomastia after a year on finasteride that needed surgery[9].

Warning signs: a lump, swelling or tenderness behind the nipple, or nipple discharge. Report any of these to a doctor promptly.

What to do if side effects appear

  1. Write it down. Note what you feel, when it started and how often. This helps a doctor separate side effects from stress, sleep or other causes.
  2. Do not self-adjust. Changing to every other day or cutting tablets without advice is common in threads[2], but it makes it harder to know what is causing what.
  3. See a doctor. A dermatologist consult costs about ₱400 to ₱1,000, and many hospitals cover it through an HMO[12]. A urologist can help with ED. One 35-year-old's urologist told him to stop[13].
  4. Stop at once for red flags. Mood changes, thoughts of self-harm, breast lumps or nipple discharge.
  5. Ask about alternatives. Topical finasteride, minoxidil alone, or other options.

Topical finasteride as an option

Topical finasteride is applied to the scalp. In a phase III trial it grew hair against placebo, with blood levels of finasteride far lower than the oral tablet[14]. Lower is not zero, so side effects remain possible[5]. It is the most common next step for Filipino men who had sides on the pill, yet some have never heard of it[13]. See the topical finasteride guide.

Questions people ask

How common are finasteride side effects?

In the 1mg trials, decreased libido was reported by 1.8% on finasteride against 1.3% on placebo, and ED by 1.3% against 0.7%[1]. Most resolved after stopping.

Are finasteride side effects permanent?

In trials, sexual side effects resolved in men who stopped and in many who continued[1]. The label also lists post-approval reports of problems that persisted after stopping. Talk to a doctor if symptoms do not settle.

Does finasteride cause infertility?

It can lower semen volume and sperm counts in some men, and these recovered after stopping in studies[6]. Men trying to conceive should discuss it with a doctor.

What does ED mean?

ED is erectile dysfunction: trouble getting or keeping an erection firm enough for sex[3].

Is topical finasteride safer than oral?

It produces much lower blood levels in trials[14], which is why some men switch after side effects. It is still a prescription drug and side effects remain possible.
