Topical finasteride: how it compares with the pill, and how to get it in the Philippines
Topical finasteride is finasteride in a spray or solution applied to the scalp instead of swallowed. In a phase III trial it grew more hair than placebo at 24 weeks, with blood levels of finasteride far lower than the 1mg tablet[1]. Lower is not zero, so the same side effects are possible, and in the Philippines it still needs a prescription.
What topical finasteride is
It is the same drug as the tablet, dissolved in a liquid you spray or drop onto the thinning areas. The idea is simple: put the drug where the follicles are and keep less of it in the rest of the body. Oral finasteride lowers DHT throughout the body[2]. Topical finasteride aims to lower it mostly in the scalp.
In the Philippines you will see it in three forms: compounded solutions made by some dermatologists, products prescribed by telehealth brands, and imported combination topicals that mix finasteride with minoxidil. That third group needs care. The PH FDA has warned against an unregistered "Hair Tonic 20 Minoxidil + Finasteride" product[3]. What the evidence shows
The key study is a phase III randomized trial of a finasteride spray in men with male pattern hair loss. After 24 weeks, the spray improved target-area hair counts significantly more than placebo, and the gain was similar to oral finasteride 1mg used as a comparison arm[1].
The bigger difference was in the blood. Plasma finasteride levels with the spray were more than 100 times lower than with the tablet, and the drop in blood DHT was smaller[1]. That is the reason doctors consider it for men who had side effects on the pill.
| Oral 1mg tablet | Topical spray or solution | |
|---|---|---|
| Hair results | Strong two-year trial data[4] | Similar to oral at 24 weeks in one phase III trial[1] |
| Blood levels | Full systemic dose[2] | Much lower[1] |
| Side effects | Sexual sides in about 1 to 2% in trials[2] | Possible, reported less often in the trial[1] |
| Daily routine | One tablet | Applied to dry scalp, left to absorb |
| Long-term data | Many years | Limited |
Not side-effect-free
Some finasteride still reaches the bloodstream, so sexual side effects, mood changes and breast tenderness are still possible. Users on r/PhHairloss have posted threads about sexual side effects on low-strength topical finasteride[5]. The pattern in Filipino threads is that some men feel better after switching from oral[6], and some do not.
Topical products can also cause scalp itch, redness or flaking. Alcohol-based liquids are more likely to irritate. Spreading the solution onto a partner or child through a pillow or hands is a theoretical concern, so wash your hands after applying and let it dry. Women who are or may become pregnant should avoid contact with finasteride[2].
Full numbers for oral sides, fertility and mood are in the finasteride side effects guide, including the fertility and mood sections. All treatment fears in one place: side effects hub.
How it is used
In the phase III trial, men sprayed the solution onto the thinning areas of a dry scalp once a day[1]. Compounded products in the Philippines vary in strength and base, so follow the instructions of the doctor who prescribed yours, not a forum thread.
- Apply to a dry scalp, not wet hair, so the solution stays where it is needed.
- Let it dry before lying on a pillow or putting on a cap. In PH heat this can take longer.
- Wash your hands after applying, especially before touching a partner or child.
- Store it away from children and pets, and check the expiry on compounded bottles, which is often shorter than on factory-made products.
- Take monthly photos in the same light. The trial judged results at 24 weeks[1], so give it time before deciding.
If you also use minoxidil, ask your doctor whether to apply them at different times or as one combined solution. Using two separate liquids twice a day is the routine many men drop first.
Who switches to topical, and why
In Filipino threads, the usual exit after oral side effects is topical finasteride, not quitting treatment entirely. Yet awareness is patchy. A 35-year-old told by his urologist to stop oral finasteride had never heard of the topical form[7].
- Men who had sexual or mood side effects on the tablet and want to stay on a DHT blocker.
- Men who are anxious about systemic effects and prefer to start with lower blood levels.
- Men already applying topical minoxidil, who can use a combined solution if a doctor prescribes one.
Women should not use topical finasteride during pregnancy or when they may become pregnant[2]. For female pattern loss, see the female hair loss guide instead.
How to get it in the Philippines
- See a dermatologist. A consult costs about ₱400 to ₱1,000, often free with an HMO at an accredited hospital[8]. Ask whether they compound topical finasteride or can name a registered product.
- Telehealth. GoRocky lists an oral and a topical hair option behind a doctor-reviewed assessment, without naming the ingredients or prices on its homepage[9]. Compare services on our online hair loss doctors page.
- Avoid unregistered imports. Check any combo tonic on the FDA verification portal first. Our legit check guide shows how.
Topical finasteride price: [TODO: price for topical finasteride in the Philippines]. For comparison, branded oral 1mg Stercia runs about ₱1,800 a month[10]. See the prices page for every option side by side.
Topical finasteride is usually paired with minoxidil, which works by a different route. If DHT blocking is not enough, some derms discuss dutasteride. The pillar guide is finasteride in the Philippines, and the full ladder is in hair loss treatment.

Questions people ask
Does topical finasteride work as well as oral?
In one phase III trial, hair count gains at 24 weeks were similar to oral finasteride 1mg and clearly better than placebo[1]. Long-term data are thinner than for the tablet.
Does topical finasteride have side effects?
Yes. Blood levels are much lower than with the tablet[1], but some drug still reaches the body. Sexual side effects have been reported by users[5], as has scalp irritation.
Is topical finasteride available in the Philippines?
Through some dermatologists and telehealth brands, with a prescription[9]. Avoid unregistered imported tonics; the PH FDA has issued an advisory against one[3].
Can I mix finasteride into my minoxidil?
Combined solutions exist, but making your own from tablets is not a tested product. Ask a doctor about a properly made combined prescription.
