Finasteride and minoxidil: how they differ and why they are used together
Finasteride is a prescription tablet that lowers DHT, the hormone that shrinks follicles, while minoxidil is a scalp liquid or foam (or an off-label tablet) that keeps hairs growing longer. They work by different routes, so many dermatologists use them together for male pattern loss, and a randomized trial found the combination worked better than either alone[1]. Both work only while you keep using them.
The short version
| Finasteride | Minoxidil | |
|---|---|---|
| How it works | Blocks the enzyme that makes DHT[2] | Lengthens the hair growth phase; exact mechanism unclear[3] |
| Form | 1mg tablet (topical exists) | 5% liquid or foam; off-label tablets |
| PH access | Prescription. Mercury, Watsons, TGP, telehealth[4] | Regroe 5% over the counter; 6% tagged prescription required[5] |
| Main job | Stops or slows loss | Grows and thickens hair |
| Women | Not for women who are or may become pregnant[2] | Yes, first-line for female pattern loss[6] |
| Common side effects | Sexual sides in about 1 to 2% in trials[2] | Early shed, scalp itch; body hair with tablets[7] |
| Monthly cost | ₱1,800 a month for Stercia | ₱600 to ₱900 a month for Regroe 5% |
How each one works
Finasteride treats the cause. Male pattern hair loss is driven by DHT, which slowly shrinks follicles on the hairline and crown. Finasteride blocks the enzyme that turns testosterone into DHT[2]. In two-year trials, 83% of men on 1mg had no further loss by hair count, against 28% on placebo[8].
Minoxidil works on the follicle itself. It keeps hairs in the growth phase longer and makes thin hairs thicker, without touching DHT. In a 48-week trial, 5% topical minoxidil grew more hair than 2% and placebo[3]. Because it does nothing to DHT, the underlying loss keeps going in the background.
A useful way to think about it: finasteride is the brake, minoxidil is the fertiliser. Users who say "years of minoxidil, still the same" are often using fertiliser with no brake[9].
Using them together
In a randomized study of Chinese men with male pattern loss, oral finasteride plus topical minoxidil gave better results than either drug alone[1]. That fits the logic: one slows the loss, the other pushes regrowth. This combination is the default many dermatologists discuss for men.
- Order of starting. Some men start both at once. Others start one first so that if side effects appear, they know which drug caused them. Discuss this with your doctor.
- The shed. Minoxidil often causes extra shedding in the first weeks[7]. Starting both together can make it hard to tell what is happening. See the shed timeline.
- Topical combos. Some products mix finasteride into a minoxidil solution. The PH FDA has warned against one unregistered combo tonic[10]. Check before you buy: is it legit?
- Telehealth bundles. Dailys lists oral minoxidil plus finasteride at ₱2,997 a month, per its own site[11].
Which one first?
This is a doctor's call, but the trade-offs are clear enough to plan around:
- Early receding at the temples, under 30, worried about progression: finasteride is the drug that targets the cause. Minoxidil alone does not slow DHT-driven loss.
- Afraid of sexual side effects: minoxidil carries no sexual side effect signal. Topical finasteride is another option with lower blood levels[12]. See finasteride side effects.
- Women: minoxidil is first-line. Finasteride is not used by women who are or may become pregnant[2]. See female hair loss.
- Cats at home: topical minoxidil is dangerous to cats[13]. Oral options or careful handling matter. See minoxidil and pets.
- Hot commute, oily hair: liquid minoxidil can look greasy in PH heat[14]. Foam or oral minoxidil are alternatives; see oral minoxidil.
Side effects compared
The risks do not overlap much, which is another reason the pair is common. Finasteride's main concern is sexual: lower libido in 1.8% against 1.3% on placebo, and ED (erectile dysfunction, trouble getting or keeping an erection) in 1.3% against 0.7%[2]. Rarer reports include mood changes and breast tenderness. Details: finasteride side effects, including the mood section.
Topical minoxidil's issues are local and practical: the early shed, itch and flaking[15], a greasy look, and danger to cats. Oral minoxidil adds unwanted body hair in about 15% and, less often, lightheadedness or fluid retention[16]. Minoxidil has no known effect on fertility[17]. Finasteride can lower semen volume, reversibly in studies[18].
Common mistakes
- Quitting minoxidil at month two. That is usually the peak of the shed, not the verdict[7].
- Judging finasteride at month three. The label says benefit takes 3 months or more to appear[2]; most derms judge it at a year.
- Buying the cheapest unlabelled product. Fakes and diluted stock make it impossible to know whether the drug failed or the bottle did.
- Changing doses after reading a thread. Community self-dosing is common but untested. Talk to a doctor first.
- Treating the wrong problem. All-over shedding after illness or childbirth is often temporary; check is my hairfall normal?

Branded Stercia 1mg plus Regroe 5% from Watsons or Mercury is the most common pharmacy combination. Users who found Propecia too expensive after a year often switch to Stercia or generics rather than quit[19]. Run your own numbers with the calculator below, or see every channel on the prices page.

Ten years at the cheapest option above: ₱288,000. Prices come from user reports, brand sites and retailer pages, not quotes. A prescription and a doctor are needed for the Rx items.
Tip for keeping costs down: buy Regroe in the 120ml size if you know you will stay on it, and ask your derm whether a cheaper registered 1mg brand fits you. Avoid switching to unlabelled stock to save money.
Stopping either one
Both are maintenance treatments. After stopping finasteride, gains reverse within about 12 months[2]. After stopping minoxidil, new growth is shed over the following months[7]. Filipino users call this "maintenance na pala"[20]. Plan for the lifetime cost before you start.

Questions people ask
Can I use finasteride and minoxidil together?
Yes. Many dermatologists prescribe both, and a randomized trial found the combination worked better than either alone[1]. A doctor should confirm it suits you.
Which is better, finasteride or minoxidil?
They do different jobs. Finasteride slows DHT-driven loss[8]; minoxidil grows and thickens hair[3]. For men, they are often used together.
Is minoxidil safer than finasteride?
They have different risks. Minoxidil has no sexual side effect signal but causes an early shed and is toxic to cats[13]. Finasteride has a small rate of sexual side effects[2].
How much does finasteride plus minoxidil cost in the Philippines?
Stercia 1mg runs about ₱1,800 a month and Regroe 5% about ₱600 to ₱900 a month. Dailys sells an oral combo at ₱2,997 a month[11].
